It should be in this format:

Pokemon (Nickname)(form)@Item
___Nature        ABILITY

EVs: 252 ATC/ 252 DEF/ 4 SPE

Then a short synopsis of what you want the pokemon to do.

Only write the ability if it has more than one available ability. Only type the form if there is more than one. For any changeable things put a "/" for example
Pokemon(nickname)@ItemA/ItemB
For separate EV spreads go like this
252 DEF/252 Sp.A/4 Sp.D  ---OR--- 252 DEF/ 4Sp.A/ 252 Sp.D

Stat abbreviations:
Health= HP
Physical Attack= ATC
physical Defense= DEF
Special Attack= Sp.A
Special Defense= Sp.D
Speed= SPE
Other than that please don't type your movesets with
~T-Bolt
~EQ
~WoW
~Hp I

Type it out, so people who are reading your team for ideas or help, can understand it.
~Thunderbolt
~Earthquake
~Will 'O Wisp
~Hidden power. (ICE TYPE)

For formes, type them out all the way, by their most recent official name.
So don't type Skymin type Shaymin (Sky Forme) And don't type Shaymin-S, type Shaymin (Sky Forme) And for pokemon that have a default forme, don't type Deoxys. Type Deoxys (Regular/Balanced Forme)

Title your team by what it is.
Things like...
~Ubers Sunny Day team. (Doubles)
~Singles OU Pure Offense
~Type Exclusive: Fighting Type Team.

Now have fun, and raters don't be total douches when you are rating. Give constructive Criticism. And when you are done give suggestions or replacement movesets/pokemon and then rate the before team on a scale of your choice and then with your corrections.
